Evaluation and Planning


Procedure:

Family members are encouraged to visit program sites annually or as needed for the purpose of providing input into the consumer's annual plan. Special invitations are issued to parents when the annual action plan is being developed. Their input in this plan is valuable.

When or if challenging behaviors, significant changes in medical conditions, critical incidents, unusual occurrences or alleged abuse should occur, the responsible staff person will contact the Service Coordinator who will call or write the parent immediately to inform them of these changes and arrange a meeting if deemed necessary. Incidents reports are written by program staff to document the consumer files. The Service Coordinator will document the contact in his/her service notes.

Purpose

To establish a procedure to inform parents of consumer's activities, challenging behaviors, significant changes in medical conditions, critical incidents, unusual occurrences and alleged abuse.
