Evaluation Procedure Relating to Consumers


Procedure:

Each consumer will be assigned to a staff member who will be trained in data collection, data analysis and how to measure outcomes. This data will be ongoing in relation to all goals established for each of the consumers. Monthly monitoring provided in most instances and additional training is provided as necessary by the staff member's supervisor.

Purpose

To establish evaluation procedures which will accurately reflect outcomes.
